Centene Foster Care - Superior Health Passport
Superior’s Health Passport is a web-based, secure application built using core clinical and claims information to deliver relevant health-care information for STAR Health members, when and where it is needed. Using Health Passport, providers can improve care coordination, eliminate waste and reduce errors by gaining a better understanding of a person’s medical history and health interactions.

The Digital Design team collaboratively creates user-centered, data-driven experiences. We conduct user research, design intuitive interfaces, craft engaging content, optimize workflows, develop artifacts, and maintain robust design systems to ensure consistency and efficiency across all projects.
Design System - Fondue
At Centene, we build products that enable members connecting to their health. A simple promise with implications for consistent improvement and a commitment to innovation for the people we serve. Design system ensures consistency and scalability through reusable components and standardized governance. The Fondue Design System provides accessibility guidelines for all application development. In order to make sure you build accessible components, you will need to the accessibility guidelines for our interactive components. This includes keyboard behavior as well as the management of ARIA roles and properties.
UX Design Model
Canonical working for a design and research team involve maintaining a single source of truth for all design assets, guidelines, and decisions across the team. This approach facilitates seamless collaboration, efficient onboarding, and a clear reference for maintaining design integrity throughout the project lifecycle.
The Foster Care Journey

The Product Vision
The Next Generation Foster Care solution solves the lack of consistent, timely and accurate information about the child for caregivers, caseworkers, providers, and service coordinators by providing contextual information at each step of child’s case. We are interesting because, unlike alternatives, where each user must take up the burden of assembling a holistic picture of child’s needs and goals through multiple tools and data sources, we provide clinical, time-aware, and actionable guidance for those goals and needs that is built with care coordination in mind.
Kick off Meetings
